Aviation Patients
Dear Airman:
In order to process your aviation medical exam you need to complete the history form on the FAA's website, https://medxpress.faa.gov. Once you log onto the site, you will be emailed a username and password. You may then go online and complete the front portion of your 8500-8 history form. Upon clicking the submit button you will receive a confirmation number. You will then print off a copy of the 8500-8 (which is PDF format) and bring it to our office for your physical along with the confirmation number you received online.
Please review the attached aviation letter for specific information regarding your physical.
