Aviation Patients
Dear Airman:
In order to improve the quality you receive during your aviation medical examination, please take a few minutes to fill out the front page of the 8500-8 history form and fax it to our office 410-224-1428, attention: Brad Rogers. If there are any restrictions, or if you have a SODA, please fax supporting documents that will help us speed the process of completing your medical examination.
To obtain your 8500-8 history form, sign on to the FAA's website, https://medxpress.faa.gov . You will be emailed a username and password. You may then go online and complete the front portion of your 8500-8. Upon clicking the submit button you will receive a confirmation number. You will then print off a copy of the 8500-8 (which is PDF format) and bring it to our office for your physical, OR preferably fax it to our office ahead of time. Along with this form you will need to bring your confirmation number you received online, and we will be able to complete your physical examination and submit it to AMCD online in a timely manner.
Future versions of medsxpress will store your information so that you will only need to modify any information that has changed since last examination.
